Key Highlights This Month:  

Here are updates from the policy and finance teams working on case management redesign:

Policy Team

The case management redesign policy team meetings are scheduled for every other week.

The team will be structured in two ways

  • The full policy team
  • Smaller groups focused on case management policy and care coordination/care management

Within a given month, the full policy team will meet during one of these meetings, and the small groups will meet during the other meeting times

In October, the care coordination/case management sub group met and in the future they will review the attached draft table on care coordination. Their goal is to: (1) go through the chart and identify which services appear to be the same in terms of population served, professional delivering the service, and activities performed; and then (2) go through the chart again, but this time looking at how the services are different or can be distinguished. 

The focus of this sub group is to group will focus on cataloging current definitions, authorities, financing, activities, and intersections between care management, and care coordination services.

 

Finance Team

Attached is the final RFP that went out related to the financial analysis.

Other

Attached are the small table notes from the August forum on case management.

At our August forum we also discussed the desire to get more data on SSTS Random moments to help us plan our budgets better. Attached is the request that has been submitted to DHS related to the needs. We will update as we get information on next steps on this.
